Massage Establishment License 

Overview

The City licenses Massage Establishments to promote public health and safety. Licensees must comply with City and State standards and may provide services only in the locations listed on their approved application.

Fees & Term

• $150 annually for a Massage Establishment license.
• Licensees may perform massage services only in the specific areas approved on their application.

What to submit

• Completed Massage Establishment Packet (see link below).
• Questionnaire for each individual listed in Part C of the packet.
• Any required supporting documents noted in the packet.
• All therapists on staff must hold: a valid State license (Wis. Stat. ch. 460) and a City of New Berlin Massage Therapist license.

Applications are reviewed by the New Berlin Police Department and the Clerk’s Office; both approvals are required before issuance.

Who needs this?

Every individual performing massage/bodywork services in New Berlin must be licensed by the State of Wisconsin and must obtain a City of New Berlin Massage Therapist license, even when working inside a licensed Massage Establishment.

City Massage Therapist License

• No City fee (license issued upon verification of a valid State license).
• Submit the City Massage Therapist application and proof of State licensure.
• Provide liability insurance documentation as indicated in the form.
